Global Excellence Waiver Form & Travel Insurance Agreement

By signing this agreement, I expressly acknowledge that naturally occurring diseases and viruses may be present and actively occurring in all environments. By signing this agreement, I voluntarily assume the risk for myself and any minors traveling with me, that I or they may be exposed to or infected while traveling, and that such exposure or infection may result in personal injury, illness, permanent disability, or death.


I, for myself, and any minors traveling with me, and on behalf of my and their heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Global Excellence Inc. its officers, agents, and/or employees, from all claims, demands, losses, and liability arising out of or related to any injury, disability, death or any other loss I may suffer due to exposure.


I have read this release of liability and assumption of risk agreement, fully understand its terms, and understand that I have given up substantial rights by signing it, and signing it freely and voluntarily without any inducement.


I understand travel insurance has been offered to me for purchase to cover certain risks inherent in travel, such as supplier bankruptcy, trip interruption, and/or inability to travel due to a medical or personal emergency. Please check the appropriate box.


 

Documents & Passports

I agree that it is my responsibility to complete all safe travel protocol documentation correctly and maintain a valid passport (6 months from your return date) and follow all rules of the airlines, cruise lines, and destination entry requirements.


Felony Convictions 

I have been advised that if I have a criminal record opportunities for entering another country might be restricted.


Many countries deny entry if you have a felony conviction. Canada, for example, is one country that won't let foreign travelers visit with a DUI on their record, even a misdemeanor DUI.


Not all countries screen visitors for criminal records. Many countries do not ask for a visa when visiting. A valid US passport will allow entry without issue, even for a convicted felon. We advise you to contact the US State Department to find out if the country you would like to visit will issue you a visa as a convicted felon.


Cancellation Fee

I also agree to a $50.00 per person cancellation fee.


No additions, deletions, or changes can be made to the release form, and completing it is a requirement for receiving your travel documents.
